Most delays happen because a pathology report, imaging report or discharge summary is missing or unreadable. Prepare: the biopsy or pathology report, imaging reports with dates, recent laboratory results, discharge summaries, current prescriptions and any referral letter. Clear photographs of printed reports are acceptable if the text is fully readable.
